Above & Beyond understands that medical information about you and your health is personal. We are committed to protecting medical information about you. We create a record of the care and services you receive at Above & Beyond. We need this record to provide you with quality care and to comply with certain legal requirements.  This notice applies to all of the records of your care generated at Above & Beyond.  We are required by law to

  • Ensure that medical information that identifies you is kept private.
  • Provide you with this notice of our legal duties and privacy practices with respect to medical information about you.

Protecting Your Privacy

Above & Beyond will charge for the full appointment fee for any appointments that are cancelled with less than 24 hours notice.

If you are more than 15 minutes late for an appointment, we reserve the right to reschedule your appointment.  Late arrivals are subject to the full fee for the scheduled session.

Above & Beyond is not a Medicare provider.  Any services provided to a Medicare patient are strictly Fee for Service.  In other words, we are not able to submit to Medicare, and you are also not able to submit your physical therapy expenses to Medicare for reimbursement.


At this time we are unable to work with Automobile Accident or Woker’s Compensation claims.

Workers Comp & Auto Claims

Above and Beyond is a Fee for Service facility which requires payment when services are rendered (each visit). We will provide each client with the proper documentation to submit to insurance for reimbursement.  If you are planning to submit to your insurance company for reimbursement, you should confirm that you have "Out of Network" benefits by contacting your insurance company prior to your first appointment.

  • Why Fee for Service?
    • We would not be able to provide the high level of care and individualized treatment that we do at the rates insurance companies are currently willing to pay for physical therapy treatments.
    • At Above & Beyond you are able to spend an entire hour or more with your therapist so that you will heal faster and require fewer visits to do so.  We believe that our physical therapy services are beneficial for everyone.  Our individualized programs will work with you to provide treatments that are efficient, effective and designed to return you to the lifestyle you deserve.

  • Can I Submit My Physical Therapy Bills to My Insurance For Reimbursement?
    • This is dependent upon your insurance company and the insurance plan you have through the insurance company, but in most cases you can submit for reimbursement.  For most insurances you should be able to find a claims form on their website, print it and submit it along with your Above & Beyond invoice which will include your receipt, treatment codes and provider number which your insurance company will require to process your claim.  The amount you will be reimbursed is dependent upon your insurance company. It is highly recommended that you contact your insurance company prior to your first visit and ask about “Out-Of-Network Physical Therapy” expenses.

  • Can I See a Physical Therapist Without a Referral From My Doctor?
    • Pennsylvania is a Direct Access state for physical therapy, and Above & Beyond is a licensed Direct Access provider.  What this means to you is that you may refer yourself to Above & Beyond without a referral from your physician. Our policy is to communicate with your physician about your status whether you are or aren’t directly referred by them.  Your insurance plan may require you to have a referral or a prescription for physical therapy for the service to be covered, or they may not pay for the services.

Above & Beyond is dedicated to providing highly individualized care for patients with orthopedic injuries.  Your plan of care is arrived at through the professional assessment of your physical therapist and physician and is based on your specific functional goals.

RehabilitaTion office Policies

DME and other supplies provided to you as part of your physical therapy treatment are typically not reimbursable by insurance companies and must be paid for at the time of your therapy session.
